COVID-19 Telehealth Policy
Fees and Payment
Fees are due at the time of service during the initial evaluation. A credit card will be stored in your chart and automatically charged at the time of each follow up encounter. Electronic payment is necessary to maximize time in session.

Follow-Up Appointments
Medication Follow-Up- Follow up appointments for patients who have recently established care, started a new medication, or those with increased complexity.
- For follow-ups of higher complexity or requiring longer duration such as family sessions.

Non-empanelled Insurance Notice
Level Up Mental Health and Feldmeier Medical Incorporated are not currently affiliated with any insurance panels. If desired, Dr. Feldmeier can provide you with a Superbill that you can submit for partial reimbursement. I recommend calling or emailing your insurance provider to verify if you have this option for out of network benefits.

Our clinic uses a HIPAA compliant version of Google Voice for consultation and out of office conversations. Emergency Calls – In the case of a medical or psychiatric emergency (when a person has thoughts of harming themselves, has thoughts of harming others, or gravely disabled to the point where they absolutely cannot care for themselves) the patient should contact emergency services by dialing 911, calling the Crisis Line or by presenting to an Emergency Department. But, please do alert Dr. Feldmeier once you have brought the patient in to the ER to receive care so that he can inform the accepting doctor about the patient’s recent history, medications, or other details that may help with acute treatment.
Consultation for New Patients
If you are a new patient to the clinic and wish to set up a free consultation in order to work with Dr. Feldmeier, please call our mainline and leave your name, contact information, the name of the patient and your relationship to them if applicable (your child for example), and a brief description of the reason for seeking psychiatric consultation and services. For consultation inquiries please allow us up to 2 business days to respond, though we strive to answer within the first day. Dr. Feldmeier will call you in the afternoon after clinic hours for an approximately 15-minute call to gather more information to verify that you or your child would benefit from Psychiatric evaluation. Note that Dr. Feldmeier will not be able to determine if medications will be appropriate from this conversation.
Initial evaluation expectations
Once scheduled for an initial evaluation slot, please make sure to make it to your appointment on time in order to beat traffic for in-person appointments and to verify your ability to login to our patient portal. (NOTE: IN PERSON EVALUATIONS ARE SUSPENDED UNTIL FURTHER NOTICE WHILE THE UNITED STATES IS UNDER PUBLIC HEALTH EMERGENCY). For Telepsychiatry appointments, please log-in approximately 10 minutes prior to session and you will be prompted to wait under you are let into the session. Please call our office phone number if you are having technical difficulties. Patients who do not present for their initial evaluation will not be rescheduled for an additional appointment if they have not communicated to Dr. Feldmeier if an emergency should supersede your ability to attend your appointment.
Patient Portal
Please create an account with CHARM upon scheduling your initial intake appointment. For remote-video visits, you will be using this web-based program which is run through Zoom. Please see both CHARM and Zoom’s terms of use and policies which should be maintained regularly.
Notice of CURES monitoring
All patients of this clinic are regularly monitored via the US DOJ CURES monitoring program. When filling Schedule II Medications, your pharmacist is required to upload this information into a database which is accessible by all prescribers. This mandates that all prescribers of Schedule II medications must review CURES report upon prescribing said medication. This is done to protect medical patients from inadvertent over-prescribing of a medication which could result in harm including death.
